Include the following information in your email:
- Full name
- Contact Phone Number
- Mailing Address
- Number of Boxes per Eye or any special requests
- Credit card number, expiration date, CVV, Billing zip code (or request for a phone call or Square invoice if you do not want to include this in your email)
- Insurance Allowance
